How do I submit a referral?

Learn how to send referrals to mentorship programs using Mentor Manager

Outside Referrals

While Mentor Manager is designed to allow collaboration between state agencies and mentorship programs, third parties can also submit referrals to partner programs without needing an account.

​Referring Someone Else

To refer someone else to a mentorship program:

  1. Find your partner organization in the list of partner mentorship programs.

  2. Click on the Submit a Referral link, under your chosen partner program.

  3. Click on the Refer Someone Else tab.

  4. Fill in your own contact information so the mentorship program can reach you if they have questions about the referral.

  5. Fill out the referrals contact information. Click Add Referral Details to provide any additional context about why you’re referring this individual and any other relevant information that can help the mentorship program understand their needs.

  6. Click Submit Referral to send the referral to the mentorship program.

Referring Yourself

To refer yourself to a mentorship program:

  1. Find your partner organization in the list of partner mentorship programs.

  2. Click on the Submit a Referral link, under your chosen partner program.

  3. Click on the Refer Yourself tab.

  4. Click Add Referral Details to provide any additional context about why you’re referring yourself, what you hope to achieve, and any other relevant information that can help the mentorship program understand your needs.

  5. Click Submit Self-Referral to send your referral to the mentorship program.

Internal Referrals

Agencies that have access to a partner’s Mentor Manager portal (such as a Department of Health and Welfare) can submit referrals directly through the portal. If a case worker submits a referral directly through the portal:

  • The case worker will automatically be assigned to the case and will be able to monitor progress, read and make notes, and communicate with the mentor.

  • Department managers will be able to view the case and monitor progress in addition to all other cases assigned to case workers.

  • Department reports will include the referral in their reports, including case updates, time tracking, and other activities.

Agencies that have access to a partner’s Mentor Manager portal should always submit referrals through the portal, rather than though the outside referral form, so that they can be assigned to the cases.


Submitting a Referral

  1. Find your partner organization in the list of partner mentorship programs.

  2. Click on the Mentor Manager Portal link, under your chosen partner program.

  3. Log in to the portal.

  4. Using the sidebar, click Cases.

  5. In the top-right corner, click Submit a Referral.

  6. Fill out the referral form with the individual’s contact information and any additional context that can help the mentorship program understand their needs.

  7. Click Submit Referral to send the referral to the mentorship program.
